Report a Claim

If you are an insured and one of your employees is injured on the job, please complete an Employer’s First Report of Injury or Illness Form and transmit the form without delay to our Claims Department.

This form can be e-mailed to TStone@TBIC.com or faxed to 512-708-9487.
If your office receives notice of a claim via the telephone from an employee, please have them contact our claims department immediately.
